This list is the work of J. Michalek which was posted to the "Pretty Soldier" BSSM mailing list. This document is worthwhile in that it gives a description of the attacks which was omitted intentionally from the character profiles of the "BSSM for Neophytes" site. The following document, reposted with permission, is the exact words of J. Michalek with slight editing.
MOON PRISM POWER, MAKE UP
Usagi raises her hand, her nails are painted, and her hand glows.
She then lowers her broach to her chest, and spins around. Her
blouse forms, then her boots, then gloves, then skirt. The screen
close-ups on her face, and her tiara forms. Usagi backs up and
salutes a crescent moon.
MOON TIARA ACTION
Sailor Moon takes off her tiara and spins around, then throws
it.
MOON TIARA STARDUST
I've never seen this, but I guess it's the same as Moon
Tiara Action, except the tiara stops and emits a stream of
stardust.
MOON HEALING ESCALATION
Sailor Moon takes out her Crescent Moon Wand and inscribes a circle
with it, sending a wave of healing sparks.
MOON CRYSTAL POWER, MAKE UP
Same as Prism Power, except some of the poses are different, she
makes her crystal shine, and she has wings at the beginning of the
transformation. Also, the moon she salutes has a rose on it.
MOON PRINCESS HALATION
Sailor Moon takes out her Cutie Moon Rod and spins around with it
until it glows. Then she waves it around before pointing and
shooting a wave of crescent moons.
MOON COSMIC POWER, MAKE UP
Sailor Moon makes her heart crystal shine. The broach attaches to
her chest, and she spins with a giant heart around her. Her blouse
forms, then her gloves, then her boots. The giant heart forms her
skirt. She straightens up and her tiara forms. She backs up and
salutes a heart.
MOON SPIRAL HEART ATTACK
The crystal in the Spiral Moon Heart Rod charges up, then Sailor
Moon spins extremely fast, ending in a pose where she holds the rod
up. A giant heart shoots out.
CRISIS, MAKE UP
Sailor Moon holds up the holy grail, then flashes her new tiara and
skirt color. She poses with her arms at her sides with a butterfly
behind her.
RAINBOW MOON HEART ACHE
The same as the Heart Attack, except a stream of hearts shoot for
her rod.
MOON CRISIS, MAKE UP (With Chibi Usa)
Both Usagi's and Chibi Usa's broach crystals gleam. Then
Usagi floats with her arms out, followed by Chibi Usa dropping in
front of her in the same pose. Then Usagi's blouse forms,
followed by Chibi Usa's blouse. Then both skirts form. Then the
scene is a close-up of both heads as their tiaras form. Then both
fly towards the screne then back to salute a moon.
MOON GORGEOUS MEDITATION
After Chibimoon calls Pegasus, Sailor Moon takes her Kaleido Moon
Scope, strikes a few poses, then shoots it.
MOON ETERNAL, MAKE UP
Usagi's head appears behind a group of feathers and her
crescent symbol appears. She backs up and spreads her wings and her
blouse forms. Feathers swirl around her to form her gloves, boots,
and skirt. She then spins around and strikes an odd saluting pose
with one leg up. She salutes outer space, I think.
STARLIGHT HONEYMOON THERAPY KISS
Usagi twirls her Moon Tiera from one hand to the other, then spins
around a few times. She stops by holding it about her head and
emitting a bright light.
SILVER MOON CRYSTAL POWER KISS
I haven't seen this attack...sorry.
MERCURY POWER, MAKE UP
Ami spins her wand around herself, then spins into the energy. Her
outfit forms around her. She then strikes a pose with her elbows
in, her hands out, and her knees together.
SHABON SPRAY
Ami gathers water energy between her hands, spins around, then
throws it.
SHABON SPRAY FREEZING
Same as the Shabon Spray, but her words are different.
MERCURY STAR POWER, MAKE UP
Same as her other transformation, but the symbol for Mercury
appears in the background at the end.
SHINE AQUA ILLUSION
A drop of water hits the ground, then a stream of water surrounds a
twirling Mercury until she throws it.
MERCURY CRYSTAL POWER, MAKE UP
(Note: This description is from the Amichan No Hatsu Koi Special;
otherwise it's just like her other transformations but with the
Harp/Mercury symbol in the background.) Ami's body is encased
in water. She spins around until she disappears behind a giant
wave. She reappears in her outfit, then she strikes a defensive
stance.
MERCURY AQUA RHAPSODY
Mercury forms a giant harp made out of water and plays it. A series
of water streams shoot out.
MERCURY AQUA MIRAGE
Mercury drops a ball of water to the ground, then creates an even
bigger ball in her hands. She spins around a few times, then shoots
out a series of water balls.
MARS POWER, MAKE UP
Rei twirls around with fire blazing around her. A close-up of her
feet shows her shoes form, followed by everything else. She strikes
a pose with one hand on her hip and the other sticking outward.
FIRE SOUL
Mars brings her hands together, her index fingers in the shape of a
gun. A small flame sprouts, and she shoots a giant flame from
it.
RIN, PYOU, TOU, SHA, KAI, JIN, RETSU, SAI, ZEN. AKURYO
TAISAN!
Mars concentrates on her spirit ward, then throws it. (Note: Akuryo
taisan means "evil spirits begone")
FIRE SOUL BIRD
Same as the Fire Soul, except the flame takes the shape of a
bird.
MARS STAR POWER, MAKE UP
Same as her other transformation, except the symbol of Mars appears
in the background.
BURNING MANDALA
Mars creates circlets of fire around herself, then shoots them.
MARS CRYSTAL POWER, MAKE UP
Same as her other transformation, except the symbol of Mars and a
bow appear in the background.
MARS FLAME SNIPER
Mars creates a fire-bow and shoots a flaming arrow.
JUPITER POWER, MAKE UP
Makoto spins with lightning around her. She brings in her arms and
legs, and the lightning encases her. She appears in uniform, then
strikes a martial arts-like pose.
SUPREME THUNDER
A lightning rod extends from Jupiter's tiara, a lightning bolt
shoots into it, and she shoots it out.
SUPREME THUNDER DRAGON
Same as the Supreme Thunder, but the lightning forms into a
dragon.
JUPITER STAR POWER, MAKE UP
Same as her other transformation, except the symbol of Jupiter
appears in the background.
SPARKLING WIDE PRESURE
A lightning rod extends from Jupiter's tiara, and lightning
crackles all around her. She goes into a praying stance, then
throws a ball of lightning.
JUPITER CRYSTAL POWER, MAKE UP
Same as her other transformation, except the symbol of Jupiter and
a circlet of leaves form in the background.
JUPITER OAK EVOLUTION
Jupiter spins and a series of energy leaves shoot out
everywhere.
VENUS POWER, MAKE UP
Minako spins and whips around her pen, surrounding her in stars.
After an explosion of stars, she appears in full uniform. She spins
around a few times, then strikes a pose with one arm straight up
and the other at her waist.
CRESCENT BEAM
A crescent moon touches Venus's hand, and she shoots out a
beam.
CRESCENT BEAM SHOWER
Same as the Crescent Beam, except the beam splits into a series of
small beams.
VENUS STAR POWER, MAKE UP
Same as her other transformation, except the symbol for Venus
appears in the background.
VENUS LOVE ME CHAIN
A chain of hearts wraps around Venus, then she shoots it out.
VENUS CRYSTAL POWER, MAKE UP
Same as her other transformation, except the symbol for Venus and a
heart-shaped circlet of hearts appear in the background.
VENUS LOVE AND BEAUTY SHOCK
Venus blows a kiss, which turns into an energy heart that she
shoots.
LUNA-BALL CHANGE
Chibi Usa dribbles her Luna Ball around then bounces it into the
air. It turns into whatever she needs it to be.
MOON PRISM POWER, MAKE UP
A stream of hearts reveal Chibi Usa's eyes, then she strikes a
pose to make her blouse appear. Then her gloves, boots, and skirt
appear. Then a close-up of her head shows her tiara and hair
crystals(?) appearing. She strikes a pose with one hand up, index
finger extended, in front of a heart.
PINK SUGAR HEART ATTACK
Trying to imitate Sailor Moon, Chibi Moon spins around with her
Chibi Moon Rod then stops holding it outward. SOMETIMES little
hearts shoot out. This attack only annoys enemies, but doesn't
really hurt.
MOON CRISIS, MAKE UP
Look under Sailor Moon.
TWINKLE YELL
Chibi Moon prays by saying, "Please, Pegasus, protect
everyone's dreams." She then rings her Crystal Clarion to
summon Pegasus. (Note: In the SuperS movie, Chibimoon does the Moon
Gorgeous Meditation alongside Sailor Moon.)
URANUS PLANET POWER, MAKE UP
The symbol of Uranus spins into Haruka's transforming pen,
causes a flash of light. Haruka turns her back and brings her arms
in as she's encased in yellow light. She reappears in full
uniform as she tosses her haif back to reveal her lips being
painted. She goes back and strikes a pose with one arm down and the
other at her side.
WORLD SHAKING
Uranus gathers energy in her hand, then punches the ground, causing
an energy ball the shape of Uranus to fly forth.
SPACE SWORD BLASTER (or BREAKER or BUSTER or STRIKER)
Uranus takes out her Space Sword and emits a beam from it.
NEPTUNE PLANET POWER, MAKE UP
The symbol for Neptune spins into Michiru's transforming pen,
causing a flash of light. Michiru spins around and encases herself
in water. After a flash of light, the screen does a close-up on her
face to show her lips getting painted. She spins backward and
strikes a pose with her arms crossed and her hands upward.
DEEP SUBMERGE
A tidal wave surrounds Neptune, and she creates a giant energy ball
from it. She shoots the ball and it takes the shape of Neptune.
SUBMARINE REFLECTION
Neptune holds out her Aqua Mirror and shoots a water blast from
it.
PLUTO PLANET POWER, MAKE UP
The symbol for Pluto spins into Setsuna's hand, causing a giant
flash. She spins and surrounds herself in fog. After a flash of
light, a close-up of her head shows her lips getting painted. She
spins backward and poses, leaning on her staff.
DEAD SCREAM
Fog swirls around the Garnet Orb, Pluto whispers her attack, and
she shoots a ball of energy. The energy is shaped in the planet
Pluto.
SATURN PLANET POWER, MAKE UP
(Seen only in the Playstation or Saturn game, I forget which.) The
symbol of Saturn spins into Hotaru's hand, cause a flash of
light. Saturn appears in full uniform, slowly opens her eyes, then
spins backward to pose with her glaive tilted downward.
SILENT WALL Saturn creates an invisible wall with her
glaive.
SILENCE GLAIVE SURPRISE
Saturn releases the power of death and silence through her glaive.
However, this attack would kill Saturn as well. (Note: I believe
this is called Death Reborn Revolution in the manga.)
DEATH RIBBON REVOLUTION (MANGA ONLY)
Saturn holds up her glaive and a series of black ribbons shout out,
destroying anything in their path.
SAILOR STAR FIGHTER--Kou Seiya
FIGHTER STAR POWER, MAKE UP
Seiya is seen transforming from male to female, then strikes poses
in her uniform. (NOTE: I know the Starlights get full
transformations later, but I haven't seen them. The other two
transform the same way.)
STAR SERIOUS LASER
Star Fighter holds her Star Com(?) and shoots a laser from it.
MAKER STAR POWER, MAKE UP
See note for Seiya.
STAR GENTLE UTERUS
Star Maker holds her Star Com and shoots an energy blob from it.
(If you can think of a better description, I'd like to hear
it.)
HEALER STAR POWER, MAKE UP
See note for Seiya.
STAR SENSITIVE INFERNO
Star Healer holds her Star Com and shoots a fire blast.
